Regular and reliable attendance is a necessary component of job/position.
Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.

Qualifications:
Two years of social service-related experience.
Nature of Work This position provides consultation and referral services to individuals, families, and target groups in the community.
Works primarily within the Adult & Family Services Division with work assignments to include Care Coordination for Managed Care Organizations for individuals 65 years and older.
Examples of Work Provides assessment and care coordination for individuals 65 years and older Interviews client/caseloads to determine and identify problems or needs for service/ programs.
Develops and writes individual service plans.
Provides information about health care access and assists individuals to access Medicare and Medical Assistance funded health care services.
Provides information about managed care health benefits.
Completes required contacts and paperwork within established timelines.
Advises clients of community resources, information and referrals for services, rights and responsibilities under programs or services.
Performs related duties as assigned.
Knowledge, Skills and Abilities Required Considerable knowledge of the social sciences Knowledge of home and community-based services and supports.
