Summary:
This position is responsible for coordinating end-of life care and hospice services to patients in their home or community setting and will include providing coordination of care, admissions facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with end-of-life care.
This position participates in the RN - Friday On Call rotation, the holiday rotation and will participate in coverage for PTO taken by the On Call RN position.
Qualifications Include:
Graduate of an associate's, diploma, or baccalaureate nursing program Current RN license by applicable state requirements Current BLS for Health Care Providers or will obtain within 3 months of start date; Valid driver's license with acceptable driving record per policy Daily access to a reliable and insured vehicle.
Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, prior experience with electronic medical record systems and belief in the hospice concept.
